Why PRP Alone Doesn’t Fix Acne Scars — And How SMART PRP® Makes a Real Difference

Many patients come to me thinking that PRP (Platelet-Rich Plasma) alone will erase acne scars. While PRP can improve skin texture, reduce inflammation, and promote healing, research and clinical experience show it rarely delivers meaningful improvement for stubborn scars on its own. In this post, I explain why PRP alone is often insufficient, and how my SMART PRP® approach combines PRP with scar-targeted treatments to deliver real results for all scar types, while improving overall skin quality.

Evidence-Based Dermatology: Why Targeted Treatments Matter

At my clinic, I provide evidence-based dermatology treatments. Each plan is personalised, drawing on my NHS dermatology training and years of specialist experience. Acne scars are complex, and a generic or “one-size-fits-all” approach rarely produces significant improvement. Scientific studies show that PRP alone provides modest benefit at best for deep acne scars.

Why PRP Alone Falls Short for Acne Scarring

PRP has become a popular treatment offered by aestheticians, medspas, and some clinics for acne scars. By injecting concentrated platelets into the skin, it releases growth factors that stimulate healing and collagen production.
 
While this can improve skin texture, tone, and brightness, PRP alone cannot:
 
● Reach deep, narrow ice-pick scars 
 
● Remodel thick bands in rolling or tethered scars 
 
● Create substantial volume in depressed boxcar scars

Many patients try standalone PRP at other providers, only to notice subtle improvements in skin smoothness rather than actual scar reduction.
 
 

The SMART PRP® Approach: Why It Works

SMART PRP® is my specialist, evidence-based modification of standard PRP treatment. It is not a standalone therapy — instead, it is combined with targeted procedures to treat each type of acne scar effectively:

● Ice-pick scars: treated with TCA CROSS to stimulate collagen deep inside the scar

● Rolling scars: addressed with subcision to release tethered bands

● Boxcar and shallow atrophic scars: improved with microneedling, RF Microneedling, or fillers where appropriate

● PRP: added strategically to enhance healing, reduce inflammation, and improve overall skin quality

Most patients require 4–6 treatment sessions, spaced a few weeks apart, to achieve optimal results. This ensures all scar types are treated thoroughly, while the skin benefits from ongoing collagen stimulation and healing.

 

How the Treatment Works

1. Assessment: Each patient’s scars are classified (ice-pick, rolling, boxcar, atrophic).
 
2. Targeted therapy: Specific treatments are chosen for each scar type.
 
3. SMART PRP®: PRP is applied to accelerate healing and stimulate collagen in
surrounding skin.
 
4. Follow-up: Progress is monitored over multiple sessions (typically 4–6), allowing gradual, sustainable improvement.
 
Consistency is key. Collagen remodelling takes time, and improvements often become noticeable gradually.
